Student Health Insurance Requirement
All undergraduate students (both domestic and international), all international graduate students and any graduate student required to be enrolled under programmatic requirements (e.g., students enrolled in the Physician Assistant Program, on-campus Occupational Therapy Program, Addiction Counseling or Clinical Mental Health Counseling master’s degree programs, etc.) registered and attending Johnson & Wales University and taking credit hours are required to have health coverage.
Students are automatically enrolled in the Student Health Insurance Plan (SHIP), an affordable year-round coverage plan provided by University Health Plans and underwritten by United HealthCare.
If you wish to use an alternative health care policy, you can waive SHIP coverage prior to the invoice date. Waivers are reviewed by University Health Plans, and UHP will notify you through your JWU Wildcat email.
If you do waive the SHIP, we encourage you to ensure that your health insurance coverage meets the following criteria:
- The claims administrator of the plan is based in the U.S. and has a U.S. telephone number and address.
- The plan provides both emergency and non-emergency health care, hospitalization and mental health benefits within your local campus area.
- The plan provides inpatient and outpatient mental health care and substance abuse benefits.
- The plan provides coverage for prescription medication.
- The lifetime benefit is unlimited.
Out-of-state Medicaid health plans, state-sponsored Children’s Health Insurance Plans, HMOs, and Kaiser Permanente may not cover non-emergency care in your local campus area. Please review your specific policy information before deciding to waive the Johnson & Wales Student Health Insurance Policy offered by United HealthCare.
New Student Required Forms
Before enrolling at Johnson & Wales University, all new students must submit completed health forms the summer before you begin your studies.
Submit your forms as early as possible, because you won’t be able to receive a course schedule, register for classes or select on-campus housing until your required documentation/records have been submitted and verified.

Download and print the Immunization & Health History Form. Read all instructions carefully, as you and your medical provider must complete all documents in their entirety.
Immunization Exemption Forms
Students who need or require immunization exemptions based on religious or medical reasons should complete an applicable form (provided below) and upload it to the health portal. Providence Campus students who are in the process of acquiring their vaccinations should complete the Temporary Immunization Exemption Certificate to document scheduled appointments to receive immunizations.
Additional Instructions for Student-Athletes
In addition to the Immunization & Health History Form, student athletes must complete additional steps. Please make sure to verify your health insurance coverage for athletic participation. It is recommended that you hand in your completed Athletic Health Forms to a member of the athletic training staff when you arrive on campus. Optional State Immunization Records Import
For a service fee of $20, you can request an automatic import of your immunization records directly from your state registry — no health provider visit or paperwork needed. To proceed, log in to the health portal and go to the Immunizations tab. Note: This service is NOT available for all states. The Immunizations tab in the health portal will provide details on participating states and eligibility.
Students must log in to the health portal using their JWU Wildcat Email login and password. Once you enter the portal, upload forms, manually enter your vaccine dates, and complete your emergency contact information.
Log in to JWU’s PyraMed Student Health Portal
Note: Students can access the health portal anytime via jwuLink (must be logged in using the JWU Wildcat Email login and password). Type “health portal” in the Search bar or go to the Tools menu > Grades & Personal Info category.
Upload Instructions
- Before uploading, review all forms carefully and make sure they are fully completed, signed and dated.
- Acceptable file upload types are JPEG, TIFF, PNG or PDF only (convert files as needed).
- Each file must be less than 6MB (compress file sizes or lower resolutions as needed).
- After you log in to the health portal, select the Document Upload tab. Use the dropdown to select which form you are uploading. Then, choose files from your phone or computer and SAVE. Wait for a pop-up confirmation to ensure your upload is successful. Repeat this process for each file that you are uploading.
- You will see a “pending” message until your uploads are verified by JWU Health Services. Please allow 2-3 business days for review (students are reviewed individually in the order records were received).
- If information is missing, a message will be sent to your JWU Wildcat Email address. Please check your inbox regularly.
- If you are having upload problems, please call us at 980-598-1700 or email us at hcs.clt@jwu.edu. We can provide help or offer other ways for you to submit your documents.
Managing Your Student Medical Records
Use our online medical record system, PyraMed Student Patient Portal, to upload medical records, input vaccines, view upcoming appointments and review and print your records. Log in with your JWU credentials.
Frequently Asked Questions
You do not need school insurance or any insurance to visit health services. However, if any lab work or testing needs to be done, you will need to provide insurance for billing.
You’ll be sent a reminder through the online patient portal. On the day of your appointment, you’ll go to the 2nd floor of the Academic Center and check-in at the kiosk. A member of the nursing staff will allow your entry into the clinic. Be sure to check your secure messages for educational materials, follow-up instructions, after-visit survey, consents and other important information.

Health Services can refill most commonly prescribed medications. Please contact us at 401-598-1151 to schedule an appointment.
Health services are confidential, and we will not disclose your medical information to parents without your consent. If you would like us to speak to a parent, please contact us to obtain a written consent form for you to complete.
Health services does not give notes for illness. We recommend that you communicate with your faculty regarding missing class or lab.
Go to universityhealthplans.com and select Johnson & Wales University from the list. You’ll see a link to “Insurance ID Card” on the navigation menu.
If you’re still an active student, you can obtain this in the PyraMed Student Health Portal.
If you’re no longer enrolled at JWU, you can email healthservices.pvd@jwu.edu and request a copy. We’ll email you a consent form and will send your vaccine record after we’ve received your consent.
Insurance costs and coverage periods can be found on the University Health Plans website.
Waivers are reviewed by University Health Plans. UHP will send notification to your JWU Wildcat email.
Well-Being Courses for Students
All new-to-JWU (first-year, transfer, and graduate) students will be asked to complete four (graduate) to five (undergraduate and transfer) different online courses from Vector Solutions/EverFi over the course of their first year.
Our goal in providing these courses is to provide students with baseline expectations, information, resources and skills to help keep one another safe and supported around the topics of:
- Sexual assault and relationship violence prevention
- Alcohol and other drug use
- Mental well-being
- Diversity, inclusion and belonging
- Prescription drug safety
All second-year undergraduate students will receive one additional online module specific to sexual assault and relationship violence prevention.
